acute. normal (0R) of histological rejection features. The subset of heart trans-plant recipients with graft failure and no evidence of cellular rejection were considered to have biopsy-negative rejection. Pathological changes observed in this setting were not included in the histological grading systems for cellular rejection, and I. Transplant Heart rejection: What every physician needs to know. As is the case with all transplanted solid organs, the cardiac allograft is subject to immune-mediated rejection since the recipient’s immune system recognizes the donor heart as foreign tissue. There are two generic types of rejection: cellular rejection (the most common type) and antibody mediated… 2004 Grading System of the International Society of Heart and Lung Transplantation for Acute Cellular Rejection: Grade 0R (no acute cellular rejection) Grade 1R (mild, low grade, acute cellular rejection): interstitial and/or perivascular infiltrate with up to 1 focus of myocyte damage Antibody-mediated rejection of the transplanted heart is a recognized clinical complication and a major limitation to survival of patients who have undergone heart transplantation. Experts have now developed a new working formulation for the pathologic diagnosis, grading, and reporting of cardiac antibody-mediated rejection.

General description of procedure, equipment, technique Endomyocardial biopsy (EMB) is a procedure that percutaneously obtains small amounts of myocardial tissue for diagnostic, therapeutic, and research purposes. It is primarily used to (1) follow the transplanted heart for myocardial rejection; (2) diagnose specific inflammatory, infiltrative, or familial myocardial disorders; and (3) sample outcomes to a median survival of 10 years, rejection within the first year remains a significant problem to patient survival and to transplanted heart function. Accordingly, transplant recipients are routinely monitored for rejection by endomyocardial biopsy, an invasive and uncomfortable procedure that is not without risk. Endomyocardial biopsy has defined rejection in clinical cardiac transplantation and established a threshold for therapy. With the development of more effective immunosuppression modalities and the asymptomatic nature of most histologic rejection episodes, controversy exists regarding the need to augment immunosuppression based purely on histologic findings. Purpose. The most recent International Society for Heart and Lung Transplantation (ISHLT) biopsy scale classifies cellular and antibody-mediated rejections. However, there are cases with acute decline in left ventricular ejection fraction (LVEF ≤ 45%) but no evidence of rejection on biopsy.
Multiple serial sections stained with haematoxylin and eosin are usually adequate for the diagnosis of rejection. While endomyocardial biopsy is the criterion standard for assessing heart transplant rejection, it is limited by a high degree of interobserver variability in grading of results and potential morbidity that can occur with the biopsy procedure. Also, the severity of rejection may not always coincide with the grading of the rejection by biopsy.

Experts have now developed a new working formulation for the pathologic diagnosis, grading, and reporting of cardiac antibody-mediated rejection. Their consensus statement is published in the December issue of The Journal of Heart and Lung Transplantation and is freely available at www.jhltonline.org. To determine the interobserver variability for pathologic grading of acute rejection, 1566 biopsies from 845 subjects in the Lung Allograft Rejection Gene Expression Observational study were regraded by a pathology panel blinded to the original diagnosis and compared to the grade of acute rejection assigned by individual center pathologists.

For acute cellular rejection [ 5 ]: ● Grade 0 – No rejection In an effort to monitor rejection, we have implemented a cellular monitoring strategy aimed at identifying episodes of rejection in biopsy specimens which may evolve into higher grades of rejection.
